September 11, 2001: Looking Back and Moving Forward

Approximately two months after the September 11, 2001 terrorist attacks, I wrote an editorial in the Journal of Traumatic Stress attempting to capture my thoughts and feelings about how this tragic event had impacted our nation and speculate about what some of the long-term consequences might be.

As the 20^th anniversary of September 11th approaches, I have reflected on what I wrote then and decided it would be useful to see how well this assessment held up, and the extent to which lessons learned in responding to the September 11th terrorist attacks are still relevant today.

In this editorial, I made the following observations:

• This attack changed everything in America, including our illusion of safety from attack from those willing to kill innocent people in order to induce terror and demonstrate their hatred of our nation.
• Direct victims and survivors would face a long, difficult recovery period that would last years and, for some, a lifetime, as would first responders.
• The impact of the attack would extend far beyond the direct victims, survivors, and first responders.
• The war on terrorists and terror raised the specter of war, political instability in the Middle East, other terrorist attacks, and concerns by many immigrants and Muslim-Americans who condemn terrorist violence that their safety will be imperiled by being caught up in concerns about outsiders that were aggravated by the terrorist attacks.

I also noted that the traumatic stress field had learned a great deal about the impact of large-scale mass casualty events in the past, and highlighted the need for us to share the following information with stakeholders in a user-friendly manner:

• Help the public put the terrorist attacks and their impacts in proper perspective by emphasizing that human beings have endured unspeakable horrific events throughout recorded history, and that many victims of such tragic events transcend those events because they are courageous, resilient, and obtain social support from their family and friends.
• Inform the public that safe and effective mental health interventions for victims and survivors exist, and encourage people to use them.
• Communicate that our fears of experiencing or being killed in terrorist attacks are exaggerated and very few of us will actually be killed or injured by terrorists. People should be prudent but not let this fear dominate their lives.
• Emphasize that living our lives in constant fear and placing dramatic restrictions on our lifestyles and behavior are not healthy for societies or for the people who live in them.
• Remind people that the first casualty of war is often the ability to see the enemy as a fellow human being, but that those whose objective is to kill thousands of innocent people without warning to make a religious or political point are unlikely to be deterred by reason or understanding alone.

In retrospect, I believe that this editorial got many things right, and accurately anticipated what survivors’ needs would be and how this tragic event would dramatically change our society.

Although I anticipated some type of military action in retaliation for the attack and to address potential future attacks, I did not envision the protracted wars in Afghanistan and Iraq that have been quite challenging for our Armed Forces and veterans.

Nor did I anticipate the extent to which protective measures in our nation — designed to increase security and safety — would dramatically change many aspects of our daily life. Anyone who has flown on an airplane or attempted to visit a government building before and after September 11 knows what I am talking about!

These measures have prevented subsequent large-scale terrorist attacks on our soil, but this worthy accomplishment has come at the meaningful cost of significant limitations in our liberty and freedom.

Nor have these measures eliminated mass violence incidents from domestic terrorists. However, the key point is that life is really different now in our nation than it was 20 years ago before September 11, 2001.

Finally, we collaborated with Dr. Sandro Galea and colleagues at the New York Academy of Medicine shortly after the attacks on the World Trade Center in New York City in a series of studies that produced important findings about the mental health impact of the attack:

• Direct victims and survivors including first responders had increased risk of several mental health problems including PTSD, depression, and increased tobacco and alcohol use.
• These problems also increased among members of the community who are not direct victims.
• Most people in the community were resilient and did not develop persistent problems or got better relatively quickly, but there were many people who still had problems years later.
• Most people who developed problems did not receive mental health services either because they were reluctant to seek them out or did not have access to them.

How can we best commemorate those who were murdered, and those whose lives were directly or indirectly affected by the September 11th terrorist attacks?

In the aftermath of this attack, our nation came together and was united in a way it had not been since the attack on Pearl Harbor on December 7, 1941. There is something about having an external foe upon which to focus our fear, anger, and attention that appears to remind us that we are one people who have much more in common than the things that divide us.

At the end of the American Civil War, President Abraham Lincoln was faced with the unenviable challenge of trying to reunite the North and the South after a bitter war in which more than 600,000 people were killed. In his second inaugural address, he stated the following:

“We must not be enemies. Though passion may have strained, it must not break our bonds of affection. The mystic cords of memory will swell when again touched, as surely they will be, by the better angels of our nature.”  

President Lincoln got this exactly right. The best way to commemorate September 11, 2001 is to recapture that spirit of unity and common purpose as a nation that made us determined to overcome this life-altering adversity.

The motto of the National Mass Violence Victimization Resource Center — “Readiness, Response, Resilience, and Community” — is a direct testament to our collective, common purpose, based upon important lessons learned from September 11^th and many other mass violence incidents, to support and serve victims, survivors and communities that experience mass casualty events, for as long as is needed.

Let us summon the better angels of our nature and make a solemn commitment to never forget those who were affected by September 11^th, as well as all other survivors of mass violence and terrorism crimes.

September 11, 2001: Reflections on memorials

Creating memorials in the aftermath of mass violence incidents serve to honor those killed, victims and survivors, and loved ones so that communities never forget the senseless act of violence that shattered lives near and far.

Developing such memorials can come with many challenges, as they are both deeply personal to those directly impacted and also public arenas to be shared with the greater community. Some tell stories of the events to offer a permanent collection of history telling, while others solely offer places of contemplation and tribute to those impacted.

Trauma impacts individuals and collective communities in a multitude of ways, often resulting in more complexities to how such spaces are planned, created, and opened.

I was privileged to have the opportunity to recently visit the 9/11 Memorial Museum on a private tour when the Museum was closed, and hear the lessons leaders learned during its inception, development, and implementation. During my tour, I was in awe of the perseverance of such a large-scale undertaking.

How does one create space for an archive, for a museum, for a memorial that does it justice, in comparison to the magnitude and impact of an event that rocked the core of direct victims and their families but also the nation as a whole?

How do you hear from and integrate the voices of those impacted?

How do you create a lasting and sustainable legacy?

What narratives are essential?

The challenges are many. However, while standing near the survivor stairwell in the quiet of the 9/11 Museum, looking on at the memorial exhibition nearby and hearing stories of those that perished, the survivors, and the heroes, I was overcome with emotion. I felt sadness, grief, and anger but, at the same time, I was filled with hope, determination, and spirit. It is in all of these emotions and the actions they foster that memorials are an essential fabric of community recovery and resilience.

"Voices of September 11th began informally in October 2001 when Mary Fetchet, a social worker who lost her 24 year-old son, Brad, and Beverly Eckert, an insurance executive who lost her husband, Sean Rooney, met at a gathering of Connecticut families.

In 2002, VOICES was formed to create a new paradigm in supplying services that provided continuity of care for those impacted by traumatic event. The organization became a central clearinghouse of information to help navigate a myriad of issues and access to resources – while advocating with other family members on the behalf of those impacted.

Since its founding, VOICES has evolved to meet the ongoing needs of the 9/11 community, developing support services and educational programs based on the ongoing assessment of our members. The work of the organization has included:

• Distribution of information via emails, newsletters, and social media
• Advocacy on a wide range of issues relevant to the 9/11 community
• In-person and teleconference support groups and focus groups
• Anxiety and depression screenings
• Annual Resiliency Symposiums on September 9
• Annual Information Forums on September 10
• Annual Commemorative Luncheon on September 11
• Interfaith Services
• 9/11 Living Memorial Project
• Publication of the "Always Remember" 10th Anniversary Commemorative Book
• Publication of Preparing for After Resource Kit
• Assisting thousands of survivors and responders in accessing medical and mental health treatment through the World Trade Center Health Program

Recognizing families impacted by tragedy live around the country and around the world, in 2003, VOICES began hosting teleconference groups facilitated by social workers to connect peer-to-peer groups – family members, survivors and responders with each other.

In response to the pandemic, VOICES initiated the Coronavirus Response Program to provide support and guidance for families and communities. This initiative included COVID-19 Tip Sheets, community support groups, and webinars & workshops led by subject matter experts. Soon after, we also developed VOICES Tip Sheets Following Traumatic Events, addressing acts of violence, social or civil unrest, or domestic terrorism."
